Name of a painting of mary and jesus?

I have to write a paper for my art history cl on a painting. Our professor didn't give us the name of the painting or the artists name so I am very lost. The image is of mary holding jesus, there are two small angels holding on to her legs and two man standing on either side of her. Mary is standing on almost a pedestal with angels on it and the words "AND-SAR-FLO-FAB". If anyone knows the name and/or artist of this painting it would be very helpful! Thanks!
